  1. Codage of variables: all codages were binary except for stage grouping.
  2. Age: median age according to calculation in each centre. HR calculated in patients’ aged more than median age.
  3. CYFRA 21-1: HR affecting patients with a pretreatment serum CYFRA 21-1 level >3.6 ng ml−1.
  4. Performance status according to Zubrod ECOG system. HR affecting patients with a PS 2.
  5. Stage grouping according to the Mountain classification. This variable has been tested according to three modalities: Ia–IIb, IIIa–IIIb, and IV.
