Photomicrographs of Case 1 (PBL, panels af) and Case 5 (DLBCL, panels gi). In Case 1, medium-power view shows diffuse tumor infiltration with residual adrenal cortex at the upper field (a, H&E stain × 200), whereas high-power shows large plasmablasts with a prominent nucleolus and abundant cytoplasm (b, H&E stain × 1000). The tumor cells express CD79a (c, × 400), CD138 (hererogeneous, d × 400), and MUM1 (e, × 400), and are diffusely positive for EBER (f, × 400). Case 5 is a DLBCL with diffuse infiltration of tumor cells (g, × 200) with adrenal cortex in the upper field. The tumor cells are large with centroblastic appearance (h, × 400). FISH shows BCL6 gene translocation with dual-color break-apart probes (i).
