(a) Cytoplasmic vacuolization characterized by tumor cells with relatively small, pyknotic irregular nuclei is commonly seen in pancreatic ductal adenocarcinoma following neoadjuvant therapy (× 200). (b) The residual ductal adenocarcinoma in this patient who received neoadjuvant therapy demonstrates abundant cytoplasmic eosinophilia with bizarre, hyperchromatic nuclei (× 200). (c) The background non-neoplastic pancreas can display extensive acinar atrophy and fibrosis with residual benign ducts associated with enlargement of the islets (white arrow) (× 40).
