Relative risk estimated by ordinary logistic regression adjusting for age of the father (<20, 20–24, 25–29, 30–34, 35–39, 40–44 and ⩾45 years), sex (male/female), birth year (4-year intervals) and, for all sites combined, site (Denmark, Israel, Norway, Sweden and Western Australia).
