A new model to understand late immune responses to kidney allografts. Severe or repetitive kidney injury induces a dysfunctional repair program leading to a sustained immune response in the kidney. Over time, the local immune response leads to the recruitment and activation of donor-reactive B cell clones, which differentiate to plasma cells and produce donor-specific antibodies, further contributing to tissue injury in a deleterious feedback mechanism
