Upon initiation of GnRH agonist therapy, it mimics endogenous LHRH and stimulates the production of FSH and LH from the pituitary, resulting in an initial surge of E2 from the ovaries. With long-term administration of GnRH agonists, the LHRH receptors are downregulated and desensitized, resulting in reduced ovarian hormone production.
