The composition of the project leadership structures, sample recruitment and participation by each survey round are shown below. We invited 801 individuals to participate in the survey panel through recommendations and direct invitations from advisory board members. Participants recruited through snowball sampling received the Research Electronic Data Capture (REDCap) link (n = 24).
