Adherence to nutritional counseling was defined as returning at least 2 of 3 nutrition diaries. Adherence to exercise training was defined as having performed at least 66.7% of prescribed training duration and reaching target duration in at least 50% of weeks. Green circles and yellow triangles (adherent to the diet intervention) in the upper right quadrant (adherent to the exercise training) show the lifestyle intervention patients who were adherent and included in the original per-protocol analysis (n = 76 patients (38%)). Patients who performed more than 200% of the prescribed training time are placed at 200% with solid symbols. Original values were at 201%, 203%, 207%, 216%, 259%, 286%, 298%, 301%, 309%, 310%, 323%, 441%, 498% and 667%.
