Here, the authors show that secretory alveolar cells in the lactating gland in several species are binucleated, which increases milk production, and that binucleation is regulated by Aurora kinase-A and Polo-like kinase-1.

Preclinical research indicates that various drugs approved for indications such as hypertension and diabetes could also have potentially beneficial effects in Alzheimer's disease, and for some drugs the evidence is also supported by epidemiological data or preliminary clinical trials. This article presents a formal consensus evaluation of these drug repositioning opportunities, and highlights several compounds for which sufficient evidence is available to encourage further investigation and potential progression to clinical trials in Alzheimer's disease.

The authors report a transcriptional profile of the discrete stages of spontaneous remyelination following toxin-induced focal demyelination in rats. They find an enrichment of retinoid X receptor (RXR) signaling pathways over the course of remyelination and show that RXR activation stimulates oligodendrocyte differentiation to enhance remyelination.

Medulloblastoma is the most common malignant brain tumour in children; having assembled over 1,000 samples the authors report that somatic copy number aberrations are common in medulloblastoma, in particular a tandem duplication of SNCAIP, a gene associated with Parkinson’s disease, which is restricted to subgroup 4α, and translocations of PVT1, which are restricted to Group 3.

HuR is an RNA-binding protein that regulates myotube differentiation in vitro. Here, the authors show that the muscle-specific ablation of HuR in mice leads to enhanced endurance capacity and an increase in oxidative fibres by destabilising PGC1α-mRNA, and show that the mice are protected against cancer cachexia

Here, Florian et al. describe mixed intronic TTTTA/TTTCA expansions of various lengths in the first intron of MARCH6 as a cause of FAME3.

New EFNS–ENS guidelines provide a broad and comprehensive overview of the literature and key recommendations regarding treatment and diagnosis across a vast range of conditions associated with dementia, with the exception of Alzheimer disease, which was deliberately excluded. Although excellent, the breadth and brevity of the guidelines lead to some important omissions.

The effective assessment and treatment of pain in individuals with dementia carries substantial challenges. A recent study evaluating the presence and treatment of pain in individuals with or without dementia reveals valuable insights into analgesic prescribing practices and highlights critical factors to be addressed in dementia treatment.

To address the question of whether a recurrent tumour is genetically similar to the tumour at diagnosis, the evolution of medulloblastoma has been studied in both an in vivo mouse model of clinical tumour therapy as well as in humans with recurrent disease; targeted tumour therapies are usually based on targets present in the tumour at diagnosis but the results from this study indicate that post-treatment recurring tumours (compared with the tumour at diagnosis) have undergone substantial clonal divergence of the initial dominant tumour clone.

Clinical trials are required to ensure new treatments are safe and effective for patients. The involvement of participants in the planning and execution of clinical trials is critical not only to their success but also so that the participating communities can benefit from them.

Drug repositioning and repurposing can enhance traditional drug development efforts and could accelerate the identification of new treatments. In this Review, Ballard and colleagues highlight priority compounds for repurposing for the treatment of Alzheimer disease.
