Thiazide-like diuretics, which target the sodium-chloride cotransporter (NCC), are first-line antihypertensive medications. Here, the authors determine the structures of human NCC in complex with two widely used thiazide-like diuretics, chlorthalidone and indapamide, and identify NCC polymorphisms that differentially affect drug-specific sensitivity.
