Whole-genome sequencing provides new insights into the clonal architecture of Barrett's esophagus and esophageal adenocarcinoma

Rebecca Fitzgerald and colleagues report whole-genome sequence analyses of 23 paired samples of Barrett's esophagus and esophageal adenocarcinoma. Their analyses of the clonal architecture of these lesions shows that copy number increases and heterogeneity persists during development of esophageal adenocarcinoma.

Dynamic clonal equilibrium and predetermined cancer risk in Barrett’s oesophagus

Barrett’s oesophagus is thought to be a precursor lesion for oesophageal cancer, and predicting the benign lesions that progress to cancer is clinically important. Here, the authors use FISH to study the clonal evolution of Barrett’s oesophagus and show that genetic diversity and somatic mutations are present early in the benign disease.

Multispectral scanning during endoscopy guides biopsy of dysplasia in Barrett's esophagus

Most cases of esophageal cancer can be prevented if detected early at the precancerous high-grade dysplasia stage in patients presenting with Barrett's esophagus, a condition that almost always precedes this form of cancer. Here, Qiu and colleagues have developed a multispectral imaging system that uses endoscopic polarized scanning spectroscopy to perform rapid optical scanning and imaging of the entire esophageal surface, providing a diagnosis in near real time.

Barrett esophagus involves the replacement of normal esophageal squamous epithelium with metaplastic columnar epithelium and is a major precursor to adenocarcinoma of the esophagus. Histological evaluation of biopsy samples from the esophagus and gastroesophageal junction for the presence of goblet cells and extent of dysplasia is the mainstay of surveillance for Barrett esophagus. This Review discusses the histological features of Barrett esophagus and its neoplastic complications.
