Boys with posterior urethral valves can go on to develop valve–bladder syndrome, and are at risk of renal damage if chronic over-distension of the bladder and upper urinary tract is not alleviated. In this Review on the management of valve–bladder syndrome, the authors discuss a number of treatments available for this group of patients, and highlight the role of nocturnal bladder drainage in alleviating hydroureteronephrosis.
