Colorectal cancer after biliopancreatic diversion for obesity

Biliopancreatic diversion (BPD) for the surgical treatment of obesity consists of a distal gastrectomy with long Roux-en-Y reconstruction, with enteroenterostomy placed 50 cm proximal to the ileocecal valve, resulting in a selective and permanent limitation of intestinal absorption for fat and starches.1

Diets high in red and processed meat, overconsumption of fat and the presence of an excess of bile acid in the intestinal lumen might increase this risk of colorectal cancer, while some data support a protective effect of calcium, vitamin D and dietary fibre intake.2, 3 The large bowel of the BPD subjects receives unabsorbed fat and starches, the concentration of bile salts in the stool is sharply increased and calcium and vitamin D absorption is reduced. Therefore, BPD subjects might be at increased risk for colorectal carcinoma.
