Late-onset herpes simplex virus-associated interstitial pneumonia after allogeneic bone marrow transplantation

Interstitial pneumonia is one of the major complications following bone marrow transplantation. It occurs in 30–40% of patients undergoing allogeneic bone marrow transplantation, and is fatal in approximately 60% of patients.12 Cytomegalovirus (CMV) is a common causative pathogen in about half of the patients with interstitial pneumonia, and the majority of others are diagnosed as idiopathic. Interstitial pneumonia usually occurs within 100 days after transplant, although there are occasional cases that develop later.34 We describe a patient with late-onset interstitial pneumonia following allogeneic bone marrow transplantation, considered to be associated with HSV infection.

On 6 June 1997, a 31-year-old man with chronic myelogenous leukemia received an allogeneic bone marrow graft from his HLA-identical sister when the disease was in chronic phase. The patient was conditioned with fractionated total body irradiation (12 Gy) followed by cyclophosphamide 60 mg/kg per day for 2 days. Cyclosporine and short term methotrexate were given for prophylaxis of acute GVHD. Sixteen days after the transplant, skin eruptions were seen on his whole body. Skin biopsy findings were consistent with cutaneous GVHD. The skin eruptions dis- appeared about 1 week after starting prednisolone (1 mg/kg/day). He developed extensive chronic GVHD 5 months after the transplant, and alternate day prednisolone and cyclosporine were started. Sulfamethoxazole trimethoprim was given for prophylaxis of Pneumocystis carinii pneumonia.
