PCA3 as a Grade Reclassification Predictor

Prostate cancer antigen 3 (PCA3), a noncoding RNA overexpressed in prostate tumours, can provide prognostic information regarding potential for grade reclassification in men enrolled on active surveillance (AS) programmes, according to a recent paper in Prostate Cancer and Prostatic Diseases.

Investigators from John Hopkins assessed the clinical utility of first PCA3 (fPCA3) and subsequent PCA3 (sPCA3) scores (calculated as the ratio of PCA3 mRNA:PSA mRNA in urine, multiplied by 1,000), and ability of change in PCA3 score to predict likelihood of Gleason grade reclassification. PCA3 scores are routinely measured in men enrolled in the Johns Hopkins AS programme. The study included a total of 260 men, who had ≥2 urine samples taken ≥3 years apart, and who underwent a prostate biopsy within 6 months of each PCA3 measurement. Men who were receiving treatment with 5-α reductase inhibitors were excluded from the cohort.
