Resuscitation of artificially ventilated newborn piglets with room air (RA) or 100% (O₂) after meconium aspiration (MA)

Background/aims: We investigated whether piglets with severe MA were as efficiently resuscitated with RA as O₂. Subjects: Twenty-one 2-5d old piglets. Interventions: Randomized into: 1) hypoxia, MA and resuscitation with RA (n=8). 2) hypoxia, MA and resuscitation with O₂ (n=8). 3) control group, MA and resuscitation with RA (n=5). Hypoxia was induced by ventilation with 8% O₂. Reoxygenation with RA or O₂ was started when mean blood pressure was <20 mmHg and/or base deficit > 20 mmol/L. Meconium (pooled, rotavaporated, diluted to a mixture of 110 mg/ml and blended) was instilled intratracheally at start of reoxygenation (group 1,2). Results: Between group 1 and 2 there were no significant differences during 2 hours of resuscitation assessed by mean artery blood pressure (MABP), base deficit (BD), pulmonary pressure (PAP), plasma hypoxantine (HYP), lung compliance (C) and tidal volume (Vt). The control group showed that the meconium aspiration injured the lungs. (Table) Conclusions: Resuscitation of MA piglets with RA is as efficient as with O₂ and secures adequate oxygenation
