Screening for hepatitis C in emergency departments

A prospective, multicentre, pragmatic, randomized clinical trial (NCT04003454) evaluated the effectiveness of screening patients for hepatitis C virus (HCV) infections in emergency departments. The investigators hypothesized that non-targeted screening, in which HCV testing was offered regardless of risk, would identify more people with hepatitis C than targeted screening, in which testing was offered based on risk assessment.

The trial was conducted in three emergency departments in urban areas of the United States and included 147,498 patient visits, with participants aged 29–57 years, of whom 51.1% were men. The patients were randomly assigned to undergo either non-targeted (n = 73,847) or targeted (n = 73,651) screening for HCV infection. The primary outcome was newly diagnosed HCV infection, and secondary outcomes included components of hepatitis C management and all-cause mortality (18 months of follow-up).
