A clonally expanded nodal T-cell population diagnosed as T-cell lymphoma after CAR-T therapy

Secondary malignancies and chimeric antigen receptor (CAR)-T-derived malignant T cell transformation have been reported after CAR-T therapy. Here, the authors describe a patient with diffuse large B-cell lymphoma (DLBCL) who developed new lymphadenopathy 2.5 years after CAR-T in the context of COVID-19 infection with histopathologic features consistent with T-cell lymphoma (TCL).

Deeper response predicts better outcomes in high-risk-smoldering-myeloma: results of the I-PRISM phase II clinical trial

Depth of response (CR/VGPR, MRD-) links to better outcomes, while biochemical progression predicts end-organ damage. Higher MHC-I on tumors and greater GZMB+ T cells in clonally expanded CD8+ T cells are tied to improved outcomes.

Bortezomib and dexamethasone induction for multiple myeloma

An important phase III trial involving transplant-eligible patients with multiple myeloma compared bortezomib and dexamethasone (BzD) induction with the previous standard of care. BzD was associated with a higher overall response and a deeper response to therapy that translated to superior responses following high-dose therapy and autologous stem cell transplantation.

Setting the standard for newly diagnosed multiple myeloma

The treatment of multiple myeloma has evolved rapidly with the use of the immunomodulatory agents thalidomide and lenalidomide as well as the proteasome inhibitor bortezomib. Cavo and colleagues recently reported results of a landmark phase III study comparing thalidomide–dexamethasone with bortezomib–thalidomide–dexamethasone in newly diagnosed multiple myeloma.

Clonal hematopoiesis is associated with adverse outcomes in multiple myeloma patients undergoing transplant

Multiple myeloma (MM) is treated with induction chemotherapy, autologous stem cell transplant (ASCT) and long-term immunomodulatory drug (IMiD) maintenance. Here, the authors show that the presence of clonal haematopoiesis of indeterminate potential (CHIP) at time of ASCT is associated with adverse outcomes in MM patients.
