Abciximab delivered to the MI lesion reduces infarct size

Intracoronary administration of the antiplatelet agent abciximab reduces infarct size in patients with ST-segment elevation myocardial infarction (STEMI), but manual thrombus aspiration confers no such benefit. These findings from the international INFUSE-AMI trial were presented at the 2012 ACC Scientific Sessions and published simultaneously in the Journal of the American Medical Association.

The two strategies investigated in this trial have been widely studied, with conflicting results. Both techniques are proposed to optimize the outcomes of primary percutaneous coronary intervention (PCI) by preventing thrombus embolization and thus restoring microvascular perfusion. “All the earlier studies have limitations,” says lead investigator Gregg Stone in a press release issued by the ACC. “To clarify which approach, if any, can reduce the size of the heart attack, we studied only patients presenting within 4 h of symptom onset with a large anterior heart attack, with blockage of the largest heart artery”.
