Benefit of polypills in a low socioeconomic community in the USA

In a socioeconomically vulnerable community in Alabama, USA, the use of a polypill containing four low-dose drugs produced greater reductions in blood pressure and LDL-cholesterol level than usual care. This finding comes from a randomized, controlled, open-label trial reported in The New England Journal of Medicine.

The investigators enrolled 303 adults without cardiovascular disease; 96% were black and 75% had an annual income lower than US$15,000. Participants were assigned to receive a polypill (containing amlodipine 2.5 mg, atorvastatin 10.0 mg, hydrochlorothiazide 12.5 mg and losartan 25.0 mg) or usual care. The monthly cost of the polypill was $26. At 12 months, pill counts indicated that adherence to the polypill regimen was 86%.
