BP management with a village-doctor-led strategy

In patients with hypertension living in rural China, a village-doctor-led intervention improved blood pressure (BP) control compared with enhanced usual care, according to the findings from the China Rural Hypertension Control Project. In this open, cluster randomized trial, 33,995 individuals aged ≥40 years with untreated BP levels ≥140/90 mmHg or treated BP levels ≥130/80 mmHg were assigned to either the village-doctor-led intervention or enhanced usual care. Village doctors in China receive medical training and are experienced in the routine screening, diagnosis and treatment of hypertension. At 18 months, mean systolic BP levels decreased by 26.3 mmHg in the intervention group compared with 11.8 mmHg in the usual care group. Mean diastolic BP levels decreased by 14.6 mmHg and 7.5 mmHg, respectively. These findings demonstrate the feasibility and effectiveness of a village-doctor-led intervention for BP management in low-resource settings.
