  1. The sample sizes, participation and crude means and standard deviations (SD) for age, body mass index (BMI), serum total cholesterol and systolic and diastolic blood pressure, and the percentage of current smoking. The age range of included participants was 25–64 years.
  2. BMI = body mass index, BP = blood pressure.
  3. aParticipation rate for those who returned the questionnaire and went through the health examination.
  4. bNumber of participants included in the current analyses, i.e. participants for whom data on all risk factor profile variables (current smoking, body mass index, systolic and diastolic BP and serum total cholesterol) were available.
