A 28-year-old man with dilated peritesticular veins. The veins of left pampiniform plexus present fluid-filled anechoic tubular structures in the region of the spermatic cord on duplex ultrasound at supine position without Valsalva maneuver (arrow, left panel); some of the fluid-filled anechoic tubular structures show color Doppler flow signal on duplex ultrasound at standing position with Valsalva maneuver, and the others do not show color Doppler flow signal (right panel). The scrotum does not show edematous and thicken.
