For the whole chronic disease management delivery system, the patients input their physical status (such as blood pressure, blood sugar, etc.) into the management information system every day. According to the patient's physical data, the management information system determines whether a doctor is needed to intervene in the diagnosis and treatment. If not, it means that the patient's condition is in a normal level and can be self-managed. If a physician intervention is needed, the patient's treatment plan will need to be re-customized, such as whether to be hospitalized, changing the dosage of drugs, and improving the diet.
