A large cystic low-density lesion (approximately 14.7 × 7.2 cm) with gas density is observed, with significant enhancement of the cyst wall on contrast-enhanced imaging. Residual normal pancreatic tissue is seen at the pancreatic head and tail, with no abnormal density or enhancement. The peripancreatic fat space is poorly defined. The ROI is outlined along the cyst boundary (including walled-off necrosis area), while avoiding critical structures such as blood vessels, the common bile duct, gastrointestinal tract, peritoneum, and mesentery.
