  • Neurocognitive impairment is a frequent manifestation of systemic lupus erythematosus in pediatric patients; however, the evidence base for the epidemiology, pathogenesis, diagnosis and treatment of this phenomenon is limited. The authors of this Review article discuss the available data and highlight avenues for future research in this clinically important field.

  • Paediatric systemic lupus erythematosus (SLE) has higher morbidity and mortality than adult-onset SLE. Interestingly, a new paper reinforces these differences between adult and childhood disease, but reasons for the more severe phenotype in children are varied and poorly understood.

  • Early-onset systemic lupus erythematosus (SLE) is thought to be associated with an increased risk to cardiovascular health as well as a severe disease phenotype. As outlined in this article, inflammatory and immune abnormalities that are intrinsic to SLE, primary dyslipidemias and secondary effects of treatments such as corticosteroids, might all contribute to premature atherosclerosis.

  • Poor medication adherence and depression interfere with the management of patients with systemic lupus erythematosus, especially adolescent and young adult patients. For patients for whom clinic visits and education programs fail to improve adherence, novel social media-based interventions are demonstrating the ability to increase patient agency, self-efficacy, and adherence.

  • The improved prognosis of children with systemic lupus erythematosus over the past 50 years means that rheumatologists now focus on preventing the long-term complications associated with this disease and its treatment. As discussed in this review, a specialized multidisciplinary approach is needed to ensure optimum family education, treatment and patient outcome in pediatric systemic lupus erythematosus.

  • Systemic lupus erythematosus (SLE) is an autoimmune disease with a strong ethnic and gender bias. In a transancestral genetic association study, Langefeldet al. identify 24 novel regions associated with risk to lupus and propose a cumulative hits hypothesis for loci conferring risk to SLE.

  • ‘Patient-centered’ research has traditionally meant that researchers and clinicians design trials for the benefit of patients. By contrast, patients today are central to study design and reporting outcomes, and new research agendas recognize that patients can point the way to research questions and how to address them.
