We found it interesting to read the Review by Rakesh Aggarwal (Diagnosis of hepatitis E. Nat. Rev. Gastroenterol. Hepatol. doi:10.1038/nrgastro.2012.187),1 which provided valuable clues to our current research on the diagnosis of chronic hepatitis E.

Hepatitis E virus (HEV) is a common cause of acute viral hepatitis in developing countries, where the sanitation conditions are usually substandard. Most cases are self-limited, but some instances of chronic HEV infection occur among people who are immunodeficient, such as patients with HIV infection and recipients of an organ transplant.2 Progression of acute HEV infection to chronic hepatitis E in immunocompetent patients is rare, with only one case reported to date.3
