Postnatal changes of atrial natriuretic peptide (ANP), extracellular volume(ECV), and diuresis in a randomised controlled trial of high frequency oscillation (HFO) vs conventional ventilation in preterm infants <30 weeks GA † 962

Background. Diuresis is reduced during ventilation with high mean airway pressures (MAD). During HFO (high volume strategy) a higher MAD is used than during CV. Hypothesis. The higher MAD during HFO reduces ANP and diuresis thus attenuating the postnatal reduction of ECV. Patients and methods. 17 infants (GA 26 wks (24-29), BW 825 g (452-1370)) were randomised to HFO (Infant Star™) and 20 infants (GA 27 wks (24-29), BW 898 g (644-1490)) to CV (Sechrist™model IV-100B). ANP (RIA), ECV(sucrose dilution), diuresis and MAD were measured on day 1 and 3. Results. MAD was higher during HFO than during CV on day 1 (median 7 cm H2O (3-11) vs 4.5 cm H2O (0-8),p=0.001). Diuresis increased and ECV decreased from day 1 to 3 in both groups. ANP showed a large variation without a systematic change from day 1 to 3. Conclusion. The increase of MAD during HFO on day 1 had no negative side effects on the postnatal adaptation of ECV. (¹: median (range) ²: mean±sd) Table
