No precision psychiatry without clinical precision

We read with great interest the ECNP’s roadmap for precision psychiatry recently published in Molecular Psychiatry [1]. This ambitious initiative rightly calls for leveraging biological and digital tools to improve psychiatric care. However, we believe it overlooks a crucial question: what are the clinical manifestations that precision psychiatry aims to explain or predict? Without addressing this issue, any progress in biological modeling may rest on unstable clinical ground.
