Myocilin gene (MYOC) mutations contribute to 4% of primary open angle glaucoma and >10% of juvenile open angle glaucoma (JOAG) [1]. Aberrant myocilin proteins accumulate in trabecular meshwork (TM) cell endoplasmic reticulum leading to cell death [2, 3]. As the proximal outflow pathway is implicated in myocilin-associated glaucoma, it is cogent to choose treatments targeting the TM. We report the case of a 25-year-old female with autosomal dominant myocilin-associated JOAG treated with ab interno 360-degree trabeculotomy using the OMNI system (Sight Sciences, Menlo Park, CA).

Week two IOPs were 24 mmHg OD and 23 mmHg OS. Week five IOPs were 18 mmHg OD and 23 mmHg OS (off topical steroids). The same procedure was then performed for the left eye. Day one IOPs were 14 mmHg OD and 10 mmHg OS. At 6 months IOPs were 12 mmHg OU off medications.
