  1. aAs main part of cause of death we regarded immediate causes of death (Ia), conditions leading to cause of death (Ib), underlying cause (Ic), and further relevant conditions (II) that may have contributed to fatal outcome in analogy to the World Health Organization (WHO) guidelines. Briefly, the immediate cause of death represented the condition (disease, injury or complication) that preceded death most directly. The condition leading to the cause of death indicated a sequence with an etiological or pathological basis that prepared the way for the immediate cause of death by damage to tissues or impairment of organ function. Underlying cause was defined as the earliest condition that started the sequence between health and death.
