Positive trials in ischaemic stroke reported at ESOC 2018

The WAKE-UP trial, led by Götz Thomalla and Christian Gerloff, was designed to determine whether MRI can be used to inform the acute management of patients with an unknown time of stroke onset. Currently, these patients are ineligible for treatment with intravenous thrombolysis because guidelines recommend this treatment only when administration within 4.5 h of known stroke onset is possible. The trial builds on previous work that showed that comparison of diffusion-weighted imaging (DWI) and fluid attenuation inversion recovery (FLAIR) imaging can identify a recent stroke.

“If there’s a mismatch between a visible ischaemic lesion on DWI but no marked hyperintensity on FLAIR imaging, we can assume that the stroke has occurred recently and the patient is likely to benefit from thrombolysis,” Thomalla explained at the ESOC.
