  1. Diabetes, hypertension, smoking and body mass index (BMI) were based on available measurements from the clinical study visit closest to when participants were 55 years of age. Diabetes was defined as fasting glucose ≥ 126 mg dl−1, non-fasting glucose ≥ 200 mg dl−1, self-report of a diagnosis of diabetes by a physician or use of glucose-lowering medication. BMI was calculated as weight in kilograms divided by height in meters squared. Hypertension was defined as blood pressure ≥ 140/90 mmHg or self-reported use of blood pressure–lowering medication. Smoking status was self-reported.
