Reasons for sample reprocessing were grouped into two categories: inherently sample-related failures (light gray) that affected a single sample only and usually required collection of a new sample, and process-related failures (that is, laboratory error) that affected multiple samples at the same time and did not usually require collection of a new sample (dark gray). Sample-related failures may be reduced in the future by accessing more punches from the DBS card for DNA extraction, but may not be able to be fully eliminated. Process-related failures can be addressed with process improvement, including increased automation and staff training, and are considered largely preventable.
