  1. aASAIR estimates were obtained from Li et al., 202217. They were produced by the GBD Disease and Injury Incidence and Prevalence Collaborators and downloaded from the IHME Global Health Data Exchange (http://ghdx.healthdata.org/gbd-results-tool) on March 20, 2020 (n = 183 countries).
  2. bLipid estimates were obtained from Micha et al., 201514,15,18. They were produced by the Nutrition and Chronic Diseases Expert Group (NutriCoDE) for the 2010 Global Burden of Diseases, Injuries, and Risk Factors Study, and they used a Bayesian hierarchical imputation model harmonize these estimates from diverse source data14,15,18.
  3. cThe development level (based on per capita GNI) for each country in 2010 was calculated using the World Bank Atlas method as described in Blencowe et al., 201225 (0 = Low-income economies, 1 = Lower-middle-income, 2 = Upper-middle-income, and 3 = High-income economies).
