The circadian rhythm is the 24-hour internal clock regulated by day and night cycles. It regulates internal biological functions such as sleep, wakefulness, hormone release and body temperature to synchronize them with external environmental changes. At the molecular level, this rhythm is controlled by the transcription factors BMAL1 and CLOCK, which regulate the expression of clock-controlled genes.

Several lines of evidence support a functional role for the circadian rhythm in cardiovascular physiology and in response to cardiovascular injury. However, the molecular mechanisms that underlie this relationship were only recently discovered by Ruan, Li, Bang et al. Corresponding author Eltzschig explained that their investigation was inspired “by clinical studies showing that heart attacks in the early morning hours are more severe, have a higher fatality rate, and are associated with more severe long-term consequences such as heart failure, compared with heart attacks that happen in the evening”.
