    This special collection entitled “Prostate biopsy: evolving strategies and perspectives” gathers a selection of recent articles published in Prostate Cancer and Prostatic Diseases (PCAN), focusing on relevant and debated aspects in the field of prostate biopsy. The featured papers address timely and clinically significant topics, ranging from the role of multiparametric MRI in guiding biopsy decisions and omission strategies, to the influence of prostate volume and core number on cancer detection. Furthermore, the ongoing transition—and debate—from transrectal to transperineal approaches is well explored. Together, these contributions offer a comprehensive and up-to-date overview of current evidence, controversies, and future directions in prostate biopsy, providing valuable insights for clinicians and experts in the field.

    This special collection entitled “New trends on the management of localized prostate cancer” features articles recently published in Prostate Cancer and Prostatic Diseases (PCAN) covering some relevant and controversial topics in the context of diagnosis, staging and treatment of localized prostate cancer. The collection was performed in collaboration between editors of PCAN and members of the scientific committee of the Italian Society of Urology. The collection features recent papers by some of the top researchers in the field and will highlight some topics of particular interest for our readers: - comparison between techniques to guide prostate biopsy - role of PSMA PET/CT in the staging of prostate cancer - urinary continence after robot-assisted radical prostatectomy - sexual outcome after radical prostatectomy in MSM - single-port prostatic surgery - management of radio-recurrent prostate cancer

    Prostate cancer screening was revolutionized in the early 1990s with the wide spread availability and FDA approval of PSA testing. PSA however is not specific for clinically significant prostate cancer and while PSA testing improves prostate cancer specific mortality worries regarding how wide-spread screening could lead to overdiagnosis and overtreatment emerged. Currently prostate cancer screening is not equated with PSA screening. Rather there is a tiered approach ideally utilizing more specific biomarkers and imaging prior to initial biopsy. Moreover, personalized screening strategies which may include genetics, family history and baseline biomarkers are emerging. This collection covers recent updates and advances in prostate cancer screening and diagnosis.
