Confidence intervals were calculated using the 95% quantiles of 1000 nonparametric bootstrap replicates. Two-sided p-values were obtained from the nonparametric bootstrap by calculating the proportion of bootstrap estimates exceeding the observed effect and multiplying by two. P-values were not adjusted for multiple comparisons. Bolded P values reflect significant associations at P < 0.05.
