Summary

Activity monitoring in older adults has evolved from simple pedometers to sophisticated wearable sensors that capture patterns of walking, sitting, standing and sleep. Modern devices integrate accelerometry, heart‐rate sensing and machine‐learning algorithms to classify postures and detect transitions between activities of daily living. Real‐time feedback and automated logging facilitate self‐monitoring and goal setting, while remote data transmission enables clinicians and caregivers to track adherence and intervene when declines in mobility or prolonged sedentary behaviour are detected. By linking objective movement data with health outcomes such as balance, fall risk and cardiometabolic markers, researchers have begun to personalise interventions aimed at preserving independence, reducing hospital admissions and delaying frailty. Across diverse settings—from community centres in Europe to rural clinics in Asia—activity monitoring has demonstrated potential to inform public‐health guidelines, support rehabilitation programmes and engage older adults in behaviour change through tailored prompts and social support. Challenges remain in ensuring device usability, data security and equitable access, particularly for the oldest old and those with sensory or cognitive impairments. Continued work on sensor validation, interoperability with electronic health records and inclusion of under-represented populations will be vital to realise the full global significance and clinical application of these technologies.

Cardiometabolic health: a composite of risk factors—including blood pressure, glucose levels and lipid profile—associated with cardiovascular and metabolic disease.
