Adipose-Derived Stem Cell Therapies for Knee Osteoarthritis
Summary
Knee osteoarthritis is characterised by progressive degeneration of articular cartilage, synovial inflammation and subchondral bone remodelling, leading to pain, stiffness and impaired mobility. Conventional management—analgesia, physiotherapy, joint offloading and ultimately joint replacement—addresses symptoms but does not halt tissue deterioration. Adipose-derived stem cell therapies exploit the abundant reservoir of mesenchymal progenitors within subcutaneous fat to promote cartilage repair, modulate inflammation and support joint homeostasis. Therapeutic preparations range from minimally manipulated stromal vascular fraction, containing a mixture of stem and accessory cells, to culture-expanded adipose-derived stem cells (ASCs) and acellular derivatives such as nanofat lysate. These interventions are delivered via intra-articular injection, often in combination with platelet-rich plasma or hyaluronic acid, and act through a combination of paracrine signalling, immunomodulation and direct chondrogenic differentiation. Early-phase clinical and preclinical studies report improvements in pain scores, functional outcomes and, in some cases, radiographic or arthroscopic evidence of cartilage regeneration. However, variability in cell yield, processing methods and dosing regimens presents challenges for standardisation. Ongoing research seeks to optimise cell isolation techniques, define potency assays, elucidate mechanisms of action and establish long-term safety and efficacy in larger, controlled trials.

TGF-β–Smad2/3 Signalling: A cellular pathway whereby transforming growth factor-beta regulates gene expression to promote cartilage matrix synthesis and inhibit inflammation.
