Adipose Tissue Disorders and Quality of Life

Summary

Adipose tissue disorders encompass a spectrum of conditions in which the distribution, composition or function of fat depots is altered, leading to chronic pain, impaired mobility and psychosocial burden. Lipedema, Dercum’s disease and localized hypertrophy syndromes are characterised by disproportionate subcutaneous fat accumulation, inflammatory changes and, in some cases, fibrotic remodelling. Patients frequently experience pain, tenderness and heaviness in the affected regions, alongside fatigue, disturbed sleep and mood disturbances. Misdiagnosis as obesity or lymphatic disease is common, delaying appropriate management and compounding distress. Recent advances have shed light on the cellular heterogeneity of adipose depots, the role of immune–stromal interactions and the impact of lymphatic function on tissue homeostasis. Quality of life is profoundly affected by chronic pain, functional limitations and body image concerns, emphasising the need for early recognition, multidisciplinary care pathways and patient-centred outcome measures. Globally, these disorders impose substantial healthcare costs and highlight the importance of tailored diagnostic criteria, novel biomarkers and targeted interventions to alleviate symptoms and restore well-being.

A foundational study has characterised the lymphatic and vascular milieu in lipedema, revealing elevated circulating levels of vascular endothelial growth factor C alongside marked macrophage infiltration in skin and fat biopsies. Despite increased pro-lymphangiogenic signals, no overt morphological changes to lymphatic vessel structure were observed, indicating a preserved lymphatic network. Instead, a distinct local cytokine profile—marked by reduced VEGF-A and VEGF-D expression—suggests that altered vascular permeability and immune cell recruitment drive tissue remodelling. These findings differentiate lipedema from classical lymphedema and offer mechanistic insights for biomarker development and anti-inflammatory strategies.
