Adolescent Pregnancy and HIV Prevention in South Africa

Adolescent pregnancy in South Africa remains a critical public health concern that is deeply intertwined with the national HIV epidemic. Girls aged 15–19 years continue to experience high rates of unintended pregnancy, driven by poverty, unequal gender norms and disruptions to schooling. Early childbearing both reflects and reinforces social vulnerability: pregnant adolescents often face stigma, reduced educational opportunities and strained relationships with partners and family members. Concurrently, adolescent pregnancy is associated with elevated risk of HIV acquisition, as younger mothers frequently engage in age-disparate sexual relationships and may have limited access to prevention services. Integrated approaches combining comprehensive sexuality education, adolescent-friendly reproductive health services and biomedical tools—such as pre-exposure prophylaxis (PrEP)—are now central to national policy. Community-based interventions that involve parents, peer mentors and male partners have demonstrated improvements in contraceptive uptake and in retention along the prevention-of-mother-to-child-transmission (PMTCT) cascade. Addressing gender-based violence and shifting harmful social norms are equally important to reduce both unintended pregnancy and HIV incidence among adolescents. Lessons from South Africa inform similar multi-sectoral strategies across sub-Saharan Africa and underscore the global imperative for youth-centred, rights-based prevention models.
