Adolescent Pregnancy Outcomes and Interventions

Adolescent pregnancy remains a pressing global health concern, with young mothers facing elevated risks of maternal complications such as hypertensive disorders, anaemia and obstructed labour. Infants born to adolescents experience higher rates of preterm birth, low birth weight and neonatal mortality. The incidence and consequences of early childbearing are strongly influenced by socio-economic determinants including poverty, limited education, rural residence and gender inequality. Interventions operate across the continuum of care: preventive strategies encompass comprehensive sexuality education and youth-friendly contraceptive services; antenatal and postnatal support benefit from trauma-informed approaches and adolescent-sensitive protocols; and broader social policies target structural inequities through community engagement and social protection. A multipronged framework that integrates health-system strengthening, policy reform and tailored community interventions is essential to reduce the prevalence and ameliorate the adverse outcomes of adolescent pregnancy.

An analysis of Demographic and Health Survey data in Ethiopia between 2005 and 2016 reveals a significant decline in teenage pregnancy from 16.6% to 12.5%, corresponding to an annual reduction rate of 2.5%. Multivariate decomposition analysis attributed nearly half of this decrease to changes in educational attainment and household wealth, with increases in secondary schooling and economic status driving most of the improvement. Conversely, membership of certain religious groups was identified as a factor that hindered further reductions. These findings underscore the critical role of socio-economic progress and educational expansion in curbing adolescent pregnancy rates and guiding targeted intervention programmes.

Trauma-informed care: Health-care approach that recognises the prevalence and impact of past trauma, aiming to avoid re-traumatisation.

Postnatal care: Health services provided to mother and infant during the period immediately following birth, typically up to six weeks postpartum.
