Adoptive Immunotherapy for Cancer Treatment
Summary
Adoptive immunotherapy harnesses the patient’s own or donor-derived lymphocytes to target and eradicate malignant cells. Central to this approach are T lymphocytes that are expanded, engineered or selected ex vivo before being reinfused into lymphodepleted hosts. Techniques range from the transfer of tumour-infiltrating lymphocytes to genetically modified T cells bearing chimeric antigen receptors. Prior lymphodepletion enhances engraftment and function by eliminating regulatory and competing immune cells. Once administered, these effector cells proliferate, traffic to tumour sites and mediate cytotoxicity through antigen-specific recognition. The strategy has demonstrated remarkable efficacy in certain haematological malignancies and holds promise for solid tumours, where overcoming immunosuppressive microenvironments remains a key challenge. Recent advances focus on improving specificity, persistence and safety, as well as combining cell therapy with checkpoint blockade or cytokine support to broaden its clinical impact.

Recent studies have elucidated how viral delivery systems can potentiate adoptive T cell responses by reshaping the tumour stroma and activating alarmin pathways. One investigation showed that a replication-competent viral vector engineered to express tumour antigens provokes interleukin-33–driven mobilisation of cytotoxic T lymphocytes, as well as an inflammatory remodelling of fibroblastic stromal cells, thereby sustaining long-term tumour control in melanoma models. Another report detailed a heterologous prime-boost regimen using two distinct arenavirus-based vectors to overcome self-tolerance, yielding tumour-specific CD8+ T cell expansions that abrogate established solid tumours and confer protection against rechallenge. These findings underscore the potential of combining adoptive cell transfer with vector-mediated microenvironmental reprogramming to enhance antitumour efficacy.
