Summary
Alcohol’s relationship with cognitive ageing is complex, reflecting a balance between potential protective effects at low to moderate intake and harm at higher levels. Moderate consumption has been associated in some settings with preservation of memory, processing speed and executive capacity, possibly through vascular and anti-inflammatory pathways or the action of bioactive compounds such as polyphenols. By contrast, heavy or binge drinking is linked to accelerated brain atrophy, particularly in the hippocampus, impaired white matter integrity and faster decline in cognitive domains. Patterns of intake, beverage type and drinking trajectory over decades interact with genetic factors (for example APOE genotype), sex and comorbidities to influence individual risk. Longitudinal studies underscore the importance of distinguishing lifelong abstainers from ex-drinkers and adjusting for lifestyle confounders. Globally, these findings inform public health guidance on safe drinking thresholds in mid and later life, encourage targeted interventions for high-risk groups and support clinical efforts to detect early cognitive change in relation to alcohol use.
