Alcohol-Related Seizures and Epilepsy
Summary
Alcohol consumption can provoke seizures both in individuals with established epilepsy and in those without a prior history of the disorder. Acute intoxication depresses central nervous system function via enhancement of GABAergic inhibition and suppression of glutamatergic excitation; abrupt cessation of heavy use reverses these effects, leading to a hyperexcitable state that may precipitate withdrawal seizures. In people with epilepsy, acute heavy drinking, disrupted sleep architecture and impaired adherence to antiseizure medications further increase seizure likelihood. Genetic predisposition, particularly in those with generalised genetic epilepsy, amplifies vulnerability. Epidemiological and genetic studies indicate that even moderate habitual intake may modestly elevate long-term risk of developing epilepsy, while other lifestyle factors and comorbidities—such as depression or metabolic disturbances—interact with alcohol exposure to influence seizure threshold. Globally, alcohol-related seizures impose a substantial burden on health systems and affected individuals, emphasising the need for targeted preventive strategies, patient education on safe consumption levels and integration of alcohol-use assessment into routine epilepsy care. Advances in mechanistic understanding and risk stratification are guiding more personalised counselling and harm-reduction approaches, with implications for public health policy and clinical guidelines.
