Alpha-Fetoprotein-Producing Gastrointestinal Malignancies
Summary
Alpha-fetoprotein-producing gastrointestinal malignancies represent a rare and aggressive subset of tumours chiefly arising in the stomach and, less commonly, the oesophagus or other segments of the digestive tract. Characterised by ectopic secretion of alpha-fetoprotein (AFP), these cancers often exhibit histological features reminiscent of hepatocellular carcinoma, a phenomenon termed the hepatoid phenotype. Clinically, they are associated with early vascular invasion, a high propensity for liver metastasis and poorer survival outcomes compared with conventional gastric adenocarcinomas. Underlying mechanisms include up-regulation of angiogenic pathways, primitive enterocyte differentiation and distinct genomic alterations such as focal amplifications and somatic copy number changes. Standard treatment modalities—surgery, systemic chemotherapy and targeted biological agents—have yielded limited long-term control, prompting exploration of novel immunotherapeutic and molecularly guided strategies. A deeper understanding of biomarker profiles, tumour microenvironment interactions and genomic drivers is critical to inform risk stratification and to develop more effective personalised therapies for patients worldwide.

Recent studies have offered new insights into the molecular landscape and metastatic behaviour of AFP-producing gastric carcinoma. One investigation employed whole-exome sequencing of tumour specimens to delineate significantly mutated genes and recurrent copy number alterations. This work revealed frequent amplifications at chromosomal loci harbouring cell-cycle regulator CCNE1 and oncogene ERBB2, which correlated with aggressive clinical features and poorer survival. Functional validation in patient-derived xenograft models demonstrated that targeted inhibition of these amplified drivers could suppress tumour growth, supporting a precision-medicine approach. Another study analysed a large surgical cohort to assess the prognostic impact of preoperative AFP. Elevated serum AFP levels were shown to strongly predict the occurrence of liver metastasis and to portend reduced recurrence-free and overall survival. Immunohistochemical evaluation further linked AFP expression to markers of a primitive enterocyte phenotype, underscoring a distinct differentiation programme that may underpin metastatic propensity.

Somatic copy number alteration (SCNA): Acquired gains or losses of chromosomal segments in tumour cells that can activate oncogenes or inactivate tumour suppressors, contributing to cancer progression.
Patient-derived xenograft (PDX): An in vivo model in which human tumour tissue is implanted into immunodeficient mice to study tumour biology and evaluate therapeutic agents under near‐clinical conditions.
Angiogenesis: The physiological process by which new blood vessels form from existing vasculature, often co-opted by tumours to support growth and metastatic spread.
