Androgenetic Alopecia Management Strategies

Summary

Androgenetic alopecia, the most prevalent form of patterned hair loss in both men and women, arises from a complex interplay of genetic predisposition and androgenic hormones, chiefly dihydrotestosterone. Management strategies fall broadly into pharmacological, regenerative and physical categories. Pharmacological approaches include topical minoxidil and oral 5α-reductase inhibitors such as finasteride and dutasteride, which act to prolong anagen phase and reduce scalp dihydrotestosterone levels respectively. Regenerative medicine modalities harness autologous biologics, notably platelet-rich plasma and stem cell–derived therapies, to stimulate follicular stem cells and neovascularisation. Physical methods, particularly low-level laser therapy, employ photobiomodulation to enhance cellular proliferation in the hair follicle. Nutraceuticals and botanical extracts offer adjunctive benefits, while surgical interventions such as follicular unit transplantation remain the gold standard for advanced cases. Emerging directions include precision genetic risk prediction, combination regimens and standardisation of biologic preparation protocols. These strategies collectively aim to restore hair density, delay progression and improve patient well-being with an emphasis on safety and long-term efficacy.

Recent large-scale genomic investigations have refined our understanding of the hereditary architecture underlying androgenetic alopecia. Exome sequencing of tens of thousands of individuals has extended the allelic spectrum, uncovering novel rare-variant associations in genes linked to monogenic hair disorders and corroborating established loci involved in follicular development. Meanwhile, a genome-wide association study in over 70,000 men has identified 71 independent susceptibility loci that together explain a substantial fraction of familial risk. These loci illuminate biological pathways related to androgen metabolism, extracellular matrix remodelling and follicular stem cell regulation. Such insights not only enhance risk stratification models but also point to new molecular targets for therapeutic intervention, laying the groundwork for personalised management and the next generation of hair-restorative agents.
