Antiphospholipid Syndrome Diagnosis and Management
Summary
Antiphospholipid syndrome is an acquired autoimmune disorder characterised by recurrent arterial or venous thrombosis and pregnancy morbidity in the presence of persistent antiphospholipid antibodies. Diagnostic evaluation combines clinical criteria—such as documented thromboembolic events or adverse obstetric outcomes—with laboratory assays that detect lupus anticoagulant, anticardiolipin antibodies and anti-β2-glycoprotein I antibodies on two or more occasions spaced at least twelve weeks apart. Risk stratification incorporates antibody profile intensity and ‘triple-positive’ status, which confer higher rates of recurrence. Standard management centres on long-term anticoagulation, traditionally with vitamin K antagonists to maintain an international normalised ratio of 2.0–3.0. In patients with obstetric complications, low-dose aspirin and heparin are employed to improve live-birth rates. Emerging strategies include direct oral anticoagulants for selected thrombotic phenotypes, adjunctive therapies such as hydroxychloroquine or statins to modulate inflammation, and targeted interventions against complement activation or neutrophil extracellular trap formation. Individualised care also addresses cardiovascular risk factors, provides multidisciplinary surveillance and seeks novel biomarkers to refine prognostic assessment.

Recent studies have demonstrated that selective agonism of the adenosine A2A receptor can suppress antibody-mediated neutrophil extracellular trap formation and attenuate thrombosis in animal models of the syndrome. By potentiating cyclic AMP signalling, both specific receptor agonists and the antithrombotic agent dipyridamole reduced vascular occlusion in mice challenged with pathogenic antibodies. These findings support an anti-inflammatory therapeutic paradigm that targets innate immune pathways downstream of antiphospholipid antibody binding, offering a potential adjunct to anticoagulant regimens.
