Summary

Antiretroviral therapy (ART) has transformed HIV from a fatal illness into a manageable chronic condition by targeting multiple stages of the viral life cycle. Modern regimens combine agents from different drug classes to achieve sustained viral load suppression, preserve or restore immune function and prevent the emergence of drug resistance. Effectiveness hinges on early initiation, optimal regimen selection, adherence support and routine monitoring of viral load and CD4 cell counts. Global efforts have expanded access to fixed-dose combinations, long-acting formulations and point-of-care diagnostics, addressing barriers in resource-limited settings. Despite these advances, challenges remain in treating special populations, managing drug–drug interactions and overcoming adherence obstacles. Ongoing research seeks to refine predictive tools, improve treatment simplification and extend the benefits of ART toward universal viral suppression.

A large retrospective cohort study evaluated predictors of viral load suppression among adults on ART in Ethiopia over a five-year period. Researchers employed accelerated failure time modelling to identify demographic, clinical and programme-level factors associated with time to suppression. Key positive predictors included older age, higher baseline CD4 count, good and fair adherence categories, disclosure of HIV status and milder WHO clinical stage. By contrast, advanced disease stage, lower haemoglobin and substance use were linked to delayed suppression. The work underscores the need for tailored adherence counselling, early nutritional support and targeted interventions for younger or high-risk groups to accelerate virological control and improve long-term outcomes.
